Developing a multilevel implementation intervention to improve equitable use of medications for patients hospitalized with heart failure

Purpose: The long-term goal of this project is to advance health equity through the implementation of evidence-based care. The objective of this proposal is to use an Implementation Mapping approach to identify key determinants of guideline-directed medical therapy (GDMT) prescribing for hospitalized HFrEF patients and develop the components of a multilevel intervention to improve equitable prescribing.
