Operationalizing Patient-Reported Outcomes Measurement with EHRs in Patients with Heart Failure (PROs HF)

Purpose: The systematic measurement of patient-reported outcomes (PROs) in routine clinical practice has profound potential to accelerate patient-centered care and research, allowing providers and health care systems to focus on outcomes that matter most to patients. This study aims are to 1) operationalize collecting patient-reported outcome measurements (PROMs) from electronic health records, 2) upload PROMs into the PCORI® Common Data Model. 3) validate a simple heart failure with reduce ejection fraction phenotype to use the PROMs in future research, and 4) analyze the association of PROMs with markers of heart failure disease severity using the data available in the individual health system’s electronic health records.
