New article about the PaTH Discovery to Implementation (D2I) initiative
The PaTH Network is sharing our successes and lessons learned from our development of the Discovery to Implementation Collaboratory to engage stakeholders across our network to prioritize health care improvements. “The PaTH from Discovery to Implementation: using a PCORnet® Clinical Research Network's own research to prioritize topics for collaborative health improvement activities” was published in Learning Health Systems, in August 2025, co-authored by members of the PaTH D2I workgroup.
In the article, Autumn Boyer and co-authors detail the rationale and step-by-step preparations for the initial D2I Collaboratory held in two online sessions in January 2023. D2I draws upon the principles of Learning Health Systems (LHS), which guide improvement cycles: performance measurements are examined by collecting data, which is then analyzed to generate knowledge about a health system so that targeted improvements can be implemented. The cycle repeats as new performance metrics are collected and analyzed to inform further improvements. PaTH’s D2I Collaboratory had two goals: (a) to ensure that the health system leaders, patients, and clinicians affiliated with our partnering health systems were aware of the findings of PaTH-related research that could inform clinical and operational decisions; (b) to build from that initial set of PaTH research findings to foster the development of one or more learning communities to initiate LHS cycles at multiple PaTH sites. The Hypertension Learning Community, still active, originated in the first PaTH D2I cycle. (The PaTH Network held a second D2I event in Fall 2024 and subsequently began the Diabetes and Chronic Kidney Disease Learning Community.)
By sharing the “hows” and “whys” of the D2I initiative, PaTH hopes to inspire other health systems, as well as other PCORnet partner networks, to implement a similar engaged process to improve health and health care for their patients.
