PaTH D2I Events Successfully Identifies New Focus Area
The PaTH Clinical Research Network’s Discovery to Implementation (D2I) Collaboratory held two productive events this November to bring together a variety of voices and make progress toward improving health care. The D2I Collaboratory was launched in 2022 to bring together different perspectives and identify health care issues that need attention. This approach follows a Learning Health System model, where health care systems use real-world data to identify problems, test solutions, and improve care over time through ongoing learning. Our teams shared a summary of all published articles that reflect PaTH data over the past 21 months with patients, caregivers, researchers, clinicians, and health system leaders at our partnering institutions. They completed two brief surveys to identify the studies of highest interest. On November 1st, a teleconference event included over 75 participants—patients, caregivers, researchers, clinicians, and health system leaders—who discussed and prioritized three key topics: kidney disease and diabetes, frailty, and opioids. The second session, on November 15th, brought together 57 participants to build on those discussions, ultimately choosing kidney disease in patients with diabetes as the focus for the next improvement project. This work will be led by a new Learning Community, a group of stakeholders working together to tackle the chosen topic, with a kick-off planned for early 2025. To close out the events, participants were asked to describe their experience with the D2I sessions in one word, and the accompanying photo features a word cloud of their responses—where the larger words reflect the most common sentiments shared.
